What is Intensive Case Management?

• The AlohaCare Community Care Services program works closely with members, providers and service coordinators to find effective ways to deliver treatment
• This often involves intensive case management programs designed to give members extra support at each stage of their member experience from first contact to follow up
• Outreach - taking the first step to find and empower those who can benefit from extra care
• Case assessment - a detailed look at the challenges you face and solutions to meet them
• Case planning (service planning, care planning) - strategies to help you succeed
• Coordination with your behavioral health and primary care provider (PCP)
• Ongoing monitoring and service coordination - follow-up to keep you on track
